The dual GIP/GLP-1 agonist behind Mounjaro and Zepbound.
Last reviewed Oct 7, 2026 · How we rate evidence · Report an error
Tirzepatide is a once-weekly injection that acts on two gut-hormone receptors, GIP and GLP-1. It is FDA-approved as Mounjaro (type 2 diabetes) and Zepbound (chronic weight management and obstructive sleep apnea with obesity). In a head-to-head trial it produced more weight loss than semaglutide.
A 39-amino-acid peptide with a C20 fatty diacid at lysine-20 that binds albumin, so it lasts about 5 days. GLP-1 and GIP receptor activation lowers appetite, slows stomach emptying, and boosts insulin release when blood sugar is high.

Approved, with large trialsRegulators approved it after big randomized controlled trials (RCTs), in which people are randomly given the drug or a placebo.
FDA-approved on the basis of large phase 3 programs (SURPASS for diabetes, SURMOUNT for obesity, SURMOUNT-OSA for sleep apnea). How we rate evidence
SURMOUNT-1: about −21% average weight loss at 72 weeks on 15 mg vs about −3% on placebo.
SURMOUNT-5: −20.2% vs −13.7% with Wegovy over 72 weeks.
Large A1c reductions in type 2 diabetes (SURPASS trials).
Cut apnea events by roughly 25–30 per hour at 52 weeks in SURMOUNT-OSA; FDA-approved for this in 2024.

| Plan | Dose | Frequency | Duration | Route |
|---|---|---|---|---|
| Label start 2.5 mg is a starting dose to build tolerance, not a treatment dose. Increase by 2.5 mg every 4 weeks as tolerated. | 2.5 mg | Once weekly | 4 weeks | Subcutaneous (abdomen, thigh, upper arm) |
| Label maintenance (low) Approved maintenance doses are 5, 10 or 15 mg. | 5 mg | Once weekly | Ongoing | Subcutaneous |
| Label maximum Reached only after stepping up through 5, 7.5, 10 and 12.5 mg. | 15 mg | Once weekly | Ongoing | Subcutaneous |

At a common mix of 30 mg of Tirzepatide in 3 mL of water (10 mg/mL), these commonly reported doses come to the following on a U-100 insulin syringe (100 units = 1 mL). They are examples of the arithmetic, not recommendations.
| Reported plan | Dose | Draw to | Volume |
|---|---|---|---|
| Label start | 2.5 mg | 25 units | 0.25 mL |
| Label maintenance (low) | 5 mg | 50 units | 0.5 mL |
| Label maximum This dose is more than the syringe or cartridge holds. Use a bigger one, or less water for a stronger mix. | 15 mg | 150 units | 1.5 mL |
Concentration (mg/mL) = vial mg ÷ mL of water; units = dose (mg) ÷ concentration × 100. A different vial size or amount of water changes every number above.

Powder
Freezer / fridge
Keep sealed vial in the freezer (−20 °C) for long-term storage; fridge (2–8 °C) is fine for a few weeks. Keep dry and out of light.
After mixing
~28 days
Refrigerate at 2–8 °C. Never freeze after mixing. Keep upright, capped, and out of light.
Travel
Keep cool
Short trips at room temperature are generally fine for powder; keep mixed vials cold.
Mounjaro is approved for type 2 diabetes; Zepbound for chronic weight management and moderate-to-severe obstructive sleep apnea in adults with obesity. The FDA declared the tirzepatide shortage over in late 2024, and enforcement discretion for compounding ended in early 2025. Research vials are not the approved drug.
Regulations change often — verify with the FDA, EMA, MHRA, Health Canada, TGA and WADA before acting.
No. The compound may be the same, but research vials have no regulated manufacturing, sterility or dose checks.
The starting dose is for tolerance. Most GI side effects show up during dose increases, so the label goes up 2.5 mg at a time every 4 weeks.
The label says take it within 4 days of the missed dose; if more than 4 days have passed, skip it and resume your usual day.
It depends on the mix. At a common mix of 30 mg of Tirzepatide in 3 mL of water (10 mg/mL), 2.5 mg is 25 units (0.25 mL) on a U-100 insulin syringe. The formula is dose in mg ÷ concentration in mg/mL × 100. Change the vial size or water in the calculator and the units update. This is math, not a dose recommendation.
Bacteriostatic water (0.9% benzyl alcohol) — allows multiple draws from one vial. A common mix is 30 mg + 3 mL, which gives 10 mg/mL. Less water makes a stronger mix and fewer units per dose; more water makes a weaker mix and more units. Pick the amount that lands your dose on an easy-to-read line.
Refrigerate at 2–8 °C. Never freeze after mixing. Keep upright, capped, and out of light. Plan on about 28 days once it is mixed, and discard it sooner if it turns cloudy or has particles. The calculator shows how long a vial lasts at your dose and flags a vial that outlasts its shelf life.
Evidence rating (5/5): FDA-approved on the basis of large phase 3 programs (SURPASS for diabetes, SURMOUNT for obesity, SURMOUNT-OSA for sleep apnea).
