Key takeaways
- A new JAMA Network Open study used AI to sort nearly 150,000 Reddit posts about unapproved retatrutide, and 7,823 users described at least one side effect.
- The top complaint online was increased appetite, not the stomach upset that dominated the clinical trials.
- Some signals do line up with the human trials: a faster heart rate and skin sensitivity show up both online and in trial data.
- Users online are reporting, not proving: nobody could check what was in their vials, how much they took, or what else they were taking.
Retatrutide is Eli Lilly's experimental weekly shot that hits three hormone receptors at once. No country has approved it, yet thousands of people are buying gray-market vials and talking about it online. This week, researchers at the University of Pennsylvania published the first large look at what those users say happens to them 12.
The result is a useful reality check in both directions. Some of what users report matches the phase 3 trials closely. Some of it doesn't match at all, and the reasons why matter for anyone reading about this drug.

What users online are reporting about retatrutide
The team pulled posts from 27 subreddits, six about retatrutide and 21 about peptides or weight loss, from 2021 through 2025 2. A language model flagged posts where people described taking the drug themselves: about 13,600 users. Of those, 7,823 described at least one side effect, and the study counted 129 different ones 12.
Among those 7,823 users, the most common reports were increased appetite (17.1%), fatigue (15.2%), increased energy (12.4%) and nausea (11.6%) 2. Insomnia came up for 6.1%, a faster heart rate for 4.8%, and a racing heart for another 3.8% 2.
The authors compared this with their earlier Reddit study of semaglutide and tirzepatide. Retatrutide users talked proportionally more about appetite, sleep, heart rate and muscle changes, and less about stomach problems 1.
What the human trials actually found
The best evidence comes from the TRIUMPH phase 3 program. In TRIUMPH-2, published in The Lancet, 1,152 adults with obesity and type 2 diabetes took retatrutide or placebo for 80 weeks 34. Weight fell 11.9%, 16.8% and 18.8% on the 4, 9 and 12 mg doses, against 5.1% on placebo 4.
In that human trial, stomach effects led the list. Diarrhea hit 27–34% of people on retatrutide versus 13% on placebo, and nausea 14–28% versus 8% 4. Increased appetite was not a trial signal, which makes it the strangest finding in the Reddit data.
Two signals do overlap. Dysesthesia, a burning or tingling skin sensitivity, appeared in 4–7% of people on retatrutide in TRIUMPH-2 and up to 21% at the top dose in TRIUMPH-4, versus 1% on placebo 5. On Reddit, 2.1% described allodynia, pain from a light touch 2. And the 2023 phase 2 trial found a dose-related rise in heart rate that peaked around week 24 and then eased (background study) 6.
Why the two pictures differ
The researchers themselves say the data cannot show the drug caused any of these symptoms 1. Nobody could verify whether the vials held real retatrutide, at what purity or dose, or whether users were also taking other peptides 2.
Gray-market use also looks nothing like a trial. Trial participants step up slowly over months under medical supervision. Online, people choose their own doses, and some stack several compounds. Hunger reports could reflect underdosed or fake product, missed doses, or simply that people who are struggling post more 2.
The authors call their results hypothesis-generating: a list of questions for real studies to test, not a new side-effect profile 12. One author discloses a research grant from Novo Nordisk, a Lilly competitor; the study itself had no funding 2.
Where glucagon and the older GLP-1 drugs overlap
Retatrutide shares two of its three targets with tirzepatide: the GLP-1 and GIP receptors, which curb appetite and slow the stomach. That is where they converge, and it fits the stomach side effects seen across all three drugs.
The difference is glucagon. Glucagon is a hormone that classically raises blood sugar, but a new review in Obesity Reviews describes a wider role in energy balance and liver health 7. Because glucagon alone can push blood sugar up, most new drugs pair it with incretin signals, as retatrutide does 7.
That third target is where the drugs diverge. The site's retatrutide entry links glucagon activity to higher energy use and lower liver fat, and lists a raised resting heart rate as a trial finding. The Penn team suggests glucagon might also help explain the energy, sleep and heart rate reports online, but they say plainly this is unconfirmed 2.
| Hormone | What it does | Link to these peptides | Evidence type |
|---|---|---|---|
| GLP-1 | Curbs appetite, slows stomach, boosts insulin | Target of semaglutide, tirzepatide, retatrutide | Human clinical |
| GIP | Boosts insulin, affects fat storage | Target of tirzepatide and retatrutide | Human clinical |
| Glucagon | Raises blood sugar, energy use; liver fat | Retatrutide's third target | Human clinical plus review |
Quick hit: HHS sends a peptide access plan to the White House
On October 15, the Associated Press reported that HHS had sent a peptide "interim access policy" to the White House budget office for review 8. Two unnamed sources said it would let compounding pharmacies temporarily make some popular peptides, including BPC-157, through enforcement discretion 8.
Details are not public. FDA staff scientists had told an advisory panel in July that none of the peptides under review had been studied enough to establish safety, and most BPC-157 data comes from animals 8. Retatrutide is a separate case: it is an investigational drug that cannot legally be compounded.
How these peptides compare
| Peptide | What it is | FDA status | Evidence (1–5) | Half-life | Banned in sport |
|---|---|---|---|---|---|
| Retatrutide | GIP, GLP-1 and glucagon triple agonist | Not approved (phase 3) | 4 | About 6 days | Yes (WADA S0) |
| Tirzepatide | GIP and GLP-1 dual agonist | Approved (Mounjaro, Zepbound) | 5 | About 5 days | No (monitored) |
| Semaglutide | GLP-1 agonist | Approved (Ozempic, Wegovy) | 5 | About 7 days | No (monitored) |
| BPC-157 | Stomach-protein fragment | Not approved | 2 | Short (hours) | Yes (WADA S0) |
The bottom line
Thousands of people are already using retatrutide outside of trials, and their reports are worth studying. Where they match the trials, on heart rate and skin sensitivity, the signal looks real. Where they don't, on hunger and sleep, the gap may say as much about gray-market products as about the drug. Retatrutide remains unapproved anywhere, and research-grade vials are not the drug tested in TRIUMPH.
