A long-acting amylin analog — the 'Cagri' half of CagriSema.
Last reviewed Oct 27, 2026 · How we rate evidence · Report an error
Cagrilintide is a once-weekly version of amylin, a hormone released with insulin that signals fullness. Novo Nordisk developed it mainly to pair with semaglutide as CagriSema. On its own it causes moderate weight loss; the combination is under FDA review, with a decision expected around late 2026. Cagrilintide alone is not approved anywhere.
A 37-amino-acid modified amylin analog with a C18 fatty diacid for a long half-life. It activates amylin and calcitonin receptors in the brain, increasing fullness and slowing stomach emptying — a different pathway from GLP-1, which is why the two are combined.

Phase 2 (Lancet 2021): −10.8% at 26 weeks on 4.5 mg vs −3.0% on placebo.
REDEFINE 1: cagrilintide 2.4 mg alone gave about −11.5% at 68 weeks (vs −3.0% on placebo).
REDEFINE 1: CagriSema about −20.4% vs −14.9% for semaglutide alone over 68 weeks (−22.7% in people who stayed on treatment).
REIMAGINE 5 topline (company report, not yet peer-reviewed): CagriSema 1.0/1.0 mg about −12.4% vs −9.1% for tirzepatide 5 mg at 60 weeks.

| Plan | Dose | Frequency | Duration | Route |
|---|---|---|---|---|
| Trial-style start REDEFINE titrated to 2.4 mg over about 16 weeks (0.25 → 0.5 → 1 → 1.7 → 2.4 mg). Community users often copy this schedule. | 250 mcg | Once weekly | 4 weeks, then step up | Subcutaneous |
| Phase 3 target dose The dose used in CagriSema and the REDEFINE monotherapy arm. | 2.4 mg | Once weekly | Trials ran 68 weeks | Subcutaneous |

At a common mix of 10 mg of Cagrilintide in 4 mL of water (2.5 mg/mL), these commonly reported doses come to the following on a U-100 insulin syringe (100 units = 1 mL). They are examples of the arithmetic, not recommendations.
| Reported plan | Dose | Draw to | Volume |
|---|---|---|---|
| Trial-style start | 250 mcg | 10 units | 0.1 mL |
| Phase 3 target dose | 2.4 mg | 96 units | 0.96 mL |
Concentration (mg/mL) = vial mg ÷ mL of water; units = dose (mg) ÷ concentration × 100. A different vial size or amount of water changes every number above.

Powder
Freezer / fridge
Keep sealed vial in the freezer (−20 °C) for long-term storage; fridge (2–8 °C) is fine for a few weeks. Keep dry and out of light.
After mixing
~28 days
Refrigerate at 2–8 °C. Never freeze after mixing. Keep upright, capped, and out of light.
Travel
Keep cool
Short trips at room temperature are generally fine for powder; keep mixed vials cold.
Cagrilintide alone is investigational. Novo Nordisk filed CagriSema (cagrilintide + semaglutide) with the FDA in December 2025; a decision is expected around Q4 2026. Research vials are unregulated and are not CagriSema.
Regulations change often — verify with the FDA, EMA, MHRA, Health Canada, TGA and WADA before acting.
No. It mimics amylin, a different fullness hormone. That's why it adds to semaglutide rather than duplicating it.
No. On its own it gave roughly 11–12% weight loss in trials, versus about 15% (14.9%) for semaglutide 2.4 mg in the same study.
There is no published trial of those combinations. The only well-studied pairing is with semaglutide.
It depends on the mix. At a common mix of 10 mg of Cagrilintide in 4 mL of water (2.5 mg/mL), 250 mcg is 10 units (0.1 mL) on a U-100 insulin syringe. The formula is dose in mg ÷ concentration in mg/mL × 100. Change the vial size or water in the calculator and the units update. This is math, not a dose recommendation.
Bacteriostatic water (0.9% benzyl alcohol) — allows multiple draws from one vial. A common mix is 10 mg + 4 mL, which gives 2.5 mg/mL. Less water makes a stronger mix and fewer units per dose; more water makes a weaker mix and more units. Pick the amount that lands your dose on an easy-to-read line.
Refrigerate at 2–8 °C. Never freeze after mixing. Keep upright, capped, and out of light. Plan on about 28 days once it is mixed, and discard it sooner if it turns cloudy or has particles. The calculator shows how long a vial lasts at your dose and flags a vial that outlasts its shelf life.
Evidence rating (4/5): Phase 2 RCT of cagrilintide alone and large phase 3 RCTs (REDEFINE) with a cagrilintide-only arm. Not approved as a standalone drug.
